FERPA - Privacy Release
The Family Educational Rights and Privacy Act (FERPA) establishes certain rights for students regarding the privacy of their educational record. While parents/guardians/spouses and others may have an interest in the student’s record, access to or release of the educational record is only by written student consent.

Privacy Release (FERPA) Forms:
1. FINANCIAL RELEASE (Student Financial Aid & Student Account Services):
Charges and payments on the student’s Campus Connection Account and information and records related to the student’s finances at UND, including financial aid. Completion of this form also allows the student access to their financial/financial aid records and account information by phone and e-mail.
- The student must select a four digit Security Code, which will be required by the student or persons listed on the form at the time of the information request.
- A new form must be submitted to change the Security Code or to add or delete individual access.
- Completed forms are submitted online to Student Account Services.
2. ACADEMIC RECORDS RELEASE (Registrar’s Office):
Information directly related to a student’s academic progress, such as grades, schedules, or a class list.
The Housing Office, Dean of Student’s Office, Student Health Services, UND Counseling Center, and other individual offices have their own release forms specific to their area, completed when service occurs.
