Self-Certification Instructions
In compliance with the Federal Reserve and Department of Education rules and regulations effective February 14, 2010, students who borrow private or institutional educational loans are required to complete the Private Loan Self-Certification Form for each loan requested, before loan funds can be disbursed.
Private Educational Loans
- Lenders will send the loan applicant the Self-Certification Form. This may be incorporated into the application process, e-mailed to the applicant, or mailed to the applicant.
- The Self-Certification form may be pre-filled.
- If it is, the applicant must print out the form, sign it, and either mail or fax it to the lender to continue with the loan approval process.
- If it is not pre-filled, see the instructions below for assistance in obtaining the information needed to complete the form.
Institutional (UND) Loans
- Students borrowing nursing or medical loans from UND are also required to complete the Private Loan Self-Certification form. The form will be e-mailed to the student's U-Mail account or is available below.
- The Self-Certification form may be pre-filled.
- If it is, the applicant must print out the form, sign it, and either mail or fax it to UND Student Financial Aid to continue with the loan approval process.
- If it is not pre-filled, see the instructions below for assistance in obtaining the information needed to complete the form.
Self-Certification Form Instructions
The Self-Certification Form must be printed, signed by the loan applicant (usually the student), and either mailed or faxed to the lender (private loans) or UND Student Financial Aid (nursing or medical loans). If the Self-Certification Form is not pre-filled, here are the steps needed to complete the form.
- Sign in to CampusConnection
- Go to: Student Center > View Financial Aid
- Then, Select Aid Year
- Next, Financial Aid Summary
- Finally, review the information within your Financial Aid Summary
(A) Cost of Attendance = Estimated Financial Aid Budget
(B) Estimated Financial Assistance = Total Aid
(C) Subtract A and B
