To register with DSS, students must have a disability, which is a physical or mental impairment that substantially limits a major life activity, such as hearing, seeing, speaking, breathing, performing manual tasks, walking, caring for oneself, learning or working.

3. Submit documentation that describes your disability. We recommend you ask a professional who knows how your disability affects you to complete the Request for Documentation form. This person could be a diagnosing or treating professional, a Special Educator or a Vocational Rehabilitation Counselor. A diagnostic report or a letter on the professional's letterhead stating the diagnosis and describing the functional limitations of the disability can be substituted for the Request for Documentation. Ask the professional to follow the DSS Disability Documentation Guidelines.
