Welcome to the University of North Dakota!
Student Health Services would like to welcome you to North Dakota and the wonderful campus of UND. We look forward to serving your health care needs while you are a student here.
In order to provide you with the best care possible it is important that you take a few minutes to read through the following information and provide us with the required health information we need.
All University of North Dakota students are required to fill out the Mandatory Health History and Immunization form, along with the TB (Tuberculosis) screening form.
Please refer to the first page of instructions to assist you in filling the form out correctly. This form needs to be filled out completely and returned to Student Health Services as soon as possible so we can review the paperwork and notify you of further information needed. FORMS NEED TO BE FILLED OUT IN ENGLISH OR TRANSLATED INTO ENGLISH. Our address can be found in the upper right hand corner of the first page of the form. Forms can also be faxed to us at 701-777-4835.
Required Immunizations and Required TB Screening Form
All University of North Dakota students born after 1956 are required by ND University System policy #506.1 to provide written documentation of two (2) doses of Measles, Mumps and Rubella (MMR) or provide documentation of titers proving immunity to each disease. UND will accept official copies of immunization records issued by local health departments, physician offices or school records. The first dose of MMR must have been given after your first birthday and the following dose at least a month later.
If you need to get the MMR vaccinations prior to leaving your home country, please get the vaccinations completed at least a month before departing for the United States.
If you are not able to receive the needed vaccinations prior to leaving your home country, Student Health Services can provide the needed vaccinations once you arrive on campus. The cost of the MMR is $13.90.
All University of North Dakota students are required to fill out the mandatory Tuberculosis (TB) screening form. If any of the indicators on the form apply to you a TB test will need to be done. Please DO NOT get your TB test done prior to coming to the United States.
TB testing needs to be done within the United States and can be done at Student Health Services once you arrive on campus. The cost of the test is $10.00. A chest x-ray performed in the US will be required for anyone with a history of positive skin test.
A TB test is still required even if you have had a previous BCG vaccination.
