Health Club Credit
Health Club Credit Enrollment Process
Step 1
Visit with a UND Wellness Center employee at our Welcome Desk to complete the Health Club Credit Enrollment Paperwork.
Each person wishing to participate must bring the following documents with them to the UND Wellness Center:
- Enrollment letter from Insurance provider
- Health Plan identification card
- UND Employee ID or Wellness Center ID card
Step 2
Each person wishing to participate in the Health Club Credit program must complete a profile, authorization for release of information and the health assessment at the My Healthy Center website.
Know Your Numbers
It is helpful if you “know your numbers.” The numbers you need to know include cholesterol (HDL,LDL and total cholesterol), blood pressure, fasting blood sugar, height, weight and waist measurements.
- Go to MyHealthCenter
- Click on "Create MyHealthCenter Account"
- Enter membership information
- Complete online authorization (annual process)
- Enter e-mail address, mailing address and phone number
- Fill out your profile
- Create a username and password
- Click on "click here to begin your health assessment" and complete the entire health assessment (annual process)
Step 3
To receive your monthly credits (reimbursements) complete the online banking information at UND Wellness Center VANCO services (This link is not part of UND or BCBSND web sites. This enrollment site is the collaboration of the University of North Dakota (UND), and VANCO Services, LLC.)
You will need to the following information to enroll in this site:
- Personal information i.e., name, address, date of birth and email
- Wellness Center ID number* and Membership monthly dues.
- Health Plan Information*
- Personal banking information i.e., checking or savings account information
* More detailed explanation of these items is given on the website once you have logged on.
There will be a two month delay between the month during which you exercise and the month in which you receive the reward. For example, if you visit the Wellness Center at least 12 days in the month of February, your participation will be validated in March and your account will be credited in late March or early April. If you do not make the minimum 12 visits during a calendar month, no payment will be made. The payment of future rewards will adhere to the same schedule as above.
Banking information storage, insurance information storage and payment processing is being performed by VANCO Services, LLC. A trusted third party vendor, a PCI compliant electronic payment processor located in the Twin Cities.
