Credential File Reactivation

Fill out the form below to request a copy of all non-confidential information in your credential file to be updated and reactivated.

Files containing personal references and evaluations (information which cannot be reproduced) will be stored for 25 years after last recorded date of use. Those containing only data forms, resumes, and/or transcripts will be destroyed after two years.

The cost to activate your file is $30.00. This fee activates your file for one year.

Please submit $1.00 for each mail out request, by check or money order, payable to UND and mailed to:
