Affiliate Cards

An ID number (EMPL #) must be issued prior to receiving your U Card. To request your ID number, please print, complete and submit the form below to Human Resources, Stop 8010. Once this form has been approved by the appropriate personnel, Human Resources will forward it to the U Card Office. The U Card Office will not accept forms hand delivered by the Affiliate applying for a U Card.

There is a $20 fee for each card which must be paid at the time of card issuance by cash, check, or credit card. To receive a U Card, the Affiliate will need to provide a government issued photo ID (Acceptable forms include: Driver's license, passport, state ID Card, Tribal ID).

For all affiliates, forms are found on the Human Resources page. To access the form go to the Human Resources website and click on 'Manager's Toolbox'. Then click on 'Forms'. You will need to login with your IDM user name and password. Scroll down to 'Payroll Processing Forms' and click on 'Affiliates'. The first link is the Affiliate Application Instructions.
