Activity/Event Approval Form

To Submit the form please do one of the three methods:

1. Save the completed form to your hard drive. Right click the Activity/Event Approval form link above & select "Save As Target". Save the file in a known location & fill out the appropriate information. Finally, email it as an attachment to und.specialevents@email.und.edu, at the Safety Office.
2. Mail the completed form to:

3. Fax the completed form to: 701.777.4132

**Please note: Activity/Event Approval form MUST be submitted 2 weeks before the proposed event is to occur. UND Safety reserves the right to deny requests not received two weeks in advance.**
