Request for Exam

Many of the personal development courses require proctored exams. Exam information will be noted within your Blackboard course. If proctored exams are required for your course, you'll need to fill out a Request for Exam form based on the type of proctor you’re using.

This form is for exam requests using an in-person Proctor. Please do not complete this form until ALL lessons preceding the exam have been submitted.

Your Order Number can be found by clicking on the ‘My Grades’ link on the left in your blackboard course.

Desired Exam Date & Time:
Please allow 3-4 business days for our office to process your exam request.
(all items are mandatory)
All lessons up to the exam must be received by our office before you request your exam.
Approved proctors are the following:
  • High school principal, guidance counselor, or teacher-not related to student or preceptor
  • University or college faculty not in a culinary, nutrition or dietetics related field - not related to student or preceptor
  • Testing services for an accredited institution, e.g. a college or technical college
  • Adult learning centers
  • Facility's nurse education director
  • Librarian
  • Others (individual must be approved by the University of North Dakota)
DIETARY MANAGER TRAINING PROGRAM STUDENTS: Your proctor MAY NOT be your R.D. Preceptor, a CDM, your administrator, a registered dietitian or a relative. Your preceptor OR administrator is NOT allowed to see your exam before or after you have taken it.
Please allow enough time for first-time proctors to be verified. See our Proctor Policy.

All lessons, up to the exam being requested, must be received by our office before you request your exam.


Please read the Proctor Policy for more information concerning exams. Proctors will be contacted for verification. REMINDER: You will need to show a valid ID prior to taking your exam.
