Foster/Adopt Inquiry Intake Form

Recruitment Inquiry Intake Process

The inquiry intake form is to be completed by the licensor from the authorized licensing agency that receives the initial contact from the inquiring family. The inquiring family may be considered new if there has been no interaction with the agency in the previous six months. The licensor will submit the online form below. All inquiries for each quarter need to be submitted at quarter’s end. This will assist with accuracy in inquiry tracking and will eliminate possible duplication. Should a licensor or family determine they are better suited for another licensing agency, the licensor will make the appropriate referral.  

Instructions

Complete the below inquiry intake form for all new inquiries. Inquiries will be tracked on a quarterly basis. Therefore, all inquiries from the previous quarter will need to be submitted by April 5, July 5, October 5, and January 5. A new form does not need to be submitted if you are following up on an inquiry referred by another agency.
