Release of Information

The forms below are available for students to download, complete, and return to the Office of Students Rights & Responsibilities.

Release of Information Form

Student conduct records are maintained in the Office of Student Rights & Responsibilities. If you need your records released regarding any Code of Student Life policy violations for your personal record or to a third party (including graduate school applications, parents, potential employers, or for background check information) please complete the “Release of Information Form.”

The information provided by the Office of Student Rights & Responsibilities is separate from information related to your academic records. If you are requesting information regarding your academic record, please contact the Office of the Registrar.

Personal Advocate Release Form

Students may elect to have a personal advocate in the Code of Student Life conduct process. More information about the role of a personal advocate is available in the Code of Student Life. Please complete the “Personal Advocate Release” in order for information to be exchanged and/or released between the Office of Student Rights & Responsibilities, you, and the advocate of your choice.
