How do I fix a log-in error? I am attempting to login, but received “Login failed, please try again” message.
Clear your cache and try again.
Google Chrome: Click the three dots in the upper right corner → “Delete Browsing Data” → delete your “cached images and files.”
Firefox: Click the three lines in the upper right corner → “History” → “Clear recent history” → clear “Temporary cached files and pages.”
My chair did not receive my Page 2 or Annual Evaluation. How do I submit?
“Save” does not send your documents to the next step in the workflow. To submit to the next step enter the workflow through your Home tab and “TO DO” area. You can also enter the workflow through your “Reviews” tab on the left side of the screen, then clicking on the workflow that is in your “Inbox.”
How do I fix a blank report? My Page 2 or Annual Evaluation Report is blank or not matching what I entered.
The chair, or who has your documents, must send them back to you. Enter the submission screen, click the refresh button, confirm that the “Last Updated” date is current, and resubmit.

How do I make changes to a completed Page 2?
Do not start a new Page 2. Click on your page 2 for the year you are changing, make and save any edits, then inform your chair you are ready for the revision workflow. You will receive an email when the workflow is initiated and it is ready for you to submit.
How do I fix incorrect and restricted information in Faculty Success?
Please submit the following form: Faculty Success Work Request. Allow up to 2 weeks for changes to be made.
