Documentation Guidelines

Purpose of Documentation

  1. To establish the student is considered a person with a disability and eligible for protection against discrimination on the basis of a disability.
  2. To provide supporting information regarding the impact the disability has on the student.
  3. To inform the need and rational for reasonable accommodations needed to facilitate equal access to the educational environments on a case-by-case basis.

Documentation May Include

  • A diagnosis of the disability or health condition and its severity 
  • A description of the limitations or impacts the health condition or disability has on the student and what barriers it may create
  • Information supporting with the disability or health condition is permanent or temporary.
    • If temporary, please provide information on the expected duration and timeline for re-evaluation of the condition.
  • Identification from a qualified healthcare provider who submitted the documentation including:
    • Contact information
    • Credentialing and Licensing Information
    • Signature
    • Date of diagnosis and most recent visit with medical provider

Documentation Formats

UND STUDENT DISABILITY RESOURCES HEALTHCARE PROVIDER FORM

UND EMOTIONAL SUPPORT ANIMAL (ESA) DOCUMENTATION REQUEST FORM  

  • Professional letter from a qualified provider on letterhead
  • Individual Education Plan (IEP) and/or 504 Plan with testing information
  • Testing Evaluations (Learning Disability, ADHD, etc.)
  • Other information from a qualified medical professional that can provide rational to eligibility of the student

How to Submit Documentation

  • The student may upload documentation through the application portal
  • The student or medical professional/provider may send via email, fax, or mail
