A medical record is maintained for every individual who receives care at Student Health Services. All information contained in your medical record is confidential.
Access to information in your file is limited to you (the patient) and authorized Student Health Services personnel, unless you provide written permission for disclosure or disclosure is required by law. All Student Health Services employees sign a confidentiality pledge to protect your privacy.
