1098-T Mail Delivery Request Form

The 1098-T tax form contains information about qualified tuition and related expenses which can be used to help determine whether the student, or the person who can claim the student as a dependent, may take either the tuition and fees deduction or claim an education credit to reduce federal income tax.

Upon completion of the Financial Obligation Agreement each semester, the student is consenting to electronic delivery of the 1098-T form, which offers expeditious access to this tax information document. Students will have the ability to view and print copies of their 1098-T tax forms online through Campus Connection. Tax information for current and prior years will be available online.

By signing this form below, you are requesting to receive the 1098-T form in the mail. It will still be available to view electronically. Once this form is submitted, the requested change will remain in effect unless the student revokes 1098-T delivery by mail.

The 1098-T form will be mailed to the student’s home address, as listed in Campus Connection. Students should review their address information in Campus Connection prior to Jan. 1 each year to ensure that it is accurate.
I have verified that my home address in Campus Connection is accurate. I understand that this request for mailing the 1098-T form will remain in effect each year, unless revoked by me.
