Important Forms
Change of Address
All F-1, J-1 and M-1 status students are required by law to report changes of address (including new residence hall rooms) to the International Student Advisors within 10 days of the move. In order to update your address, you will need to update it in Campus Connection and then email the International Student Advisors.
Request for Social Security Number-Employer Information Sheet
To apply for a Social Security Number, you must have a letter from your employer and a letter from the International Center.
Print the Information Sheet and take this to your employer to request your employer letter. Then email the employer letter to the International Center to request a letter from an International Student Advisor. Please allow 2-5 business days for processing.
Request for Reduced Enrollment (final semester only)
F-1 status students may be eligible to be less than full time during their final semester of their program. Print the Request for Reduced Enrollment form and take it to your academic advisor to be signed. Return the signed, completed form to the International Center for review via email, during walk-in hours, or by making an appointment with an International Student Advisor.
If you are not in your final semester and are seeking permission to enroll less than full time, you must email an International Student Advisor first.
You must always receive permission to reduce your enrollment before you drop classes. Failure to do so is a violation of your student status requirements. Make sure you begin this process 2-5 business days before you need to drop the course.
Request for Program Extension
Students who will not complete their coursework by the end date listed on their I-20 or DS-2019 must file for an extension in order to maintain status. Extensions may be granted when delays are caused by legitimate academic or medical circumstances. Extensions cannot be granted for solely for completion of thesis or dissertation. Extensions may be granted only if the student can prove their continued physical presence in the United States is required and the requirements cannot be met remotely. Delays due to CPT or OPT participation will not be considered.
To be considered for an extension, you must do the following:
- Complete Request for Program Extension form with academic advisor
- Gather new financial certification documents
- Send the documents to an International Student Advisor at least two weeks before your program end date
Extensions must be requested before the end date listed on the I-20 or DS-2019.
