Administrative Procedures Petition

Instructions  

This petition requests an exception to university‐wide requirements and policies and once completed and reviewed by the Office of the Registrar, will be reviewed by the University Senate Administrative Procedures Committee.  

Tuition/Fee Refund Appeals Policy

To complete the PowerSign form, please follow the instructions below:   

  1. Begin Administrative Procedures Petition
    a. Forms can only be initiated by the student.
    b.
    Students must fill in all the signers full name and UND email addresses needed for the petition (academic advisor, instructor, etc.) If marked with a * Required, those signatures are required.
         i.
    If you are petitioning for a complete withdraw after the deadline from UND, you will need the signatures of your assigned professional academic advisor.
        ii.
    If you are petitioning a withdrawal from one or more courses, (you will need a separate form for each course) but not a complete withdraw from UND, you will need the signatures of your assigned professional academic advisor and the instructor of the course you are withdrawing from.
    c.
    Once full name and UND email addresses of all signers is entered this begins the routing (signature) process.
  2. Student:  Name and email address *Required
  3. Student Professional Academic Advisor:  Enter your advisor’s full name and UND email address.  If you are unsure of your primary academic advisor, you can find it in Campus Connection or in Hawk Central. *Required
  4. Instructor of the Course: only enter the instructor’s name and UND email address if you are asking for an exception for one course. If you are unsure of your advisor’s information, you can find it is the Schedule of Classes in Campus Connection.
  5. As the student, it is your responsibility to notify the recipients of the Administrative Procedures Petition that they will be receiving an email with the document that they will need to process.   

Once completed, click on the Green Begin Signing Button at the bottom.  This will take you to the Administrative Procedures Petition Form.  Click on the green Continue Button, then click Start to complete the Administrative Procedures Petition.   

Items marked as Required need a signature from the designated individual prior to submitting the form.   

  1. Enter student information at the top of the form.
  2. Complete Action Requested section of the form, be specific. Make sure that you include any pertinent information regarding your request. For example:
    a. Withdraw from all classes after the deadline for spring 2023, drop PSYC 241 after the deadline, (class number) for summer 2023.
    b.
    Late add ENGR 450 (class number) for spring 2023, change to S/U grading from graded for HIST 104 from spring 2023, etc.
  3. Why Committee Should Approve Your Request section, be specific.  Explain in detail why the committee should approve your request.  If needed, you can attach a document to the form for further details or evidence.
  4. Click "FINISH" to begin routing. 

Once the form has been submitted to the Office of the Registrar, the form will be reviewed at the next scheduled meeting.  Once the petition has been reviewed and approved or disapproved, the student will receive an email with the outcome of the committee’s decision.   

Form Tips 

If you have any questions on how to complete this form, please email marci.mack@UND.edu or und.registrar@UND.edu.   

  1. Sign the form.
    a. Students must sign all forms, which is a separate step from initiating the form. The student will receive an email containing a link to sign the form.
    b.
    After the student signs, the form will begin automatically routing through the remaining required signers until it finishes in the Office of the Registrar.
  2. Form is finished.
  3. Once the form is finished, all parties who signed the document will receive a confirmation email with a PDF of the completed form attached. 

DocuSign Tips 

  • Always use the @UND.edu email (not @ndus.edu or personal) address for yourself and other signers.
  • Login to DocuSign using your @UND.edu login to be able to track the progress of your forms.
  • Enter your @UND.edu email address and click Next 
  • Login using your NDUS dotted ID and password (same as Blackboard and Campus Connection) on the NDUS Portal Log In page.
  • Forms cannot be corrected/changed once they have been signed and you will be required to submit a new form.
